How to Renew:
1. Login to your KBN Nurse Portal account and click the "Apply for Renewal" link next to your license information.
2. The application will ask you to upload a copy of a paystub or w2, proving you have worked at least 8 hrs as a nurse aide in the past 2 years.
 

 

 

CMA REGISTRY RENEWAL - every 1 year

How to Renew:
1. Login to your KBN Nurse Portal account and click the "Apply for Renewal" link next to your license information.
2. The application will ask you to upload copies of the following required documents:

• Annual Performance Evaluation - obtain from Crystal

• Validation of Competency Form - print a copy of this form to have Crystal sign it. CLICK HERE FOR COMPETENCY FORM

• Proof of 4 hours of medication specific training (can obtain Relias certificates from Paul)

• Proof of 40 hours worked as a medication aide in the past year (paystub, w2)

3. Confirm all of your information and complete the application.

4. Pay the renewal fee.
